Assessment Procedures

New assessment procedures -

To request any assessment, send an email to the State Director of Assessments (SDA) with the following information:

Your name -
Your current grade -
Purpose of the assessment (developmental, maintenance, or upgrade) -

Date of the match -
Time of the match -
Location & field number -
Level of play -
Your phone number -

Provide at least 4 days notice before the assessment date.
